Directly with Suncare Adult Day Care
Or with the U.S. Department of Health and Human Services (HHS)
You will not be retaliated against for filing a complaint.
We reserve the right to change this Notice at any time. Any changes will apply to all PHI we maintain and will be posted with an updated effective date.
